I'm wondering how he got his contact list to look the way he did; not the skin, but the Display picture to the left, and their name on top of Quick Message.

It's an option of the skin itself. You'll get it when you install the skin.
If you want this effect without the looks of the skin it requires messing around with the UI Files manually. Which can be a hard job, but if you're comfortable with the Plus! skinning engine you should be able to find it out by looking at his files.

Ive downloaded the skin and I'm using it right now, how do I enable it?

It should look like in the screenshot by default after you imported the skin.
Are you using Messenger version 2009?
