Which dodgy traffic? Incoming traffic can only do dodgy things if you are running dodgy software or out of date exploitable software where the traffic can connect to. And when you have a router in between (very common for broadband connections) that's only when you configured it to forward the specific port the traffic is comming from to a specific IP address within your LAN. And outgoing traffic is basically under your full control in the first place, Windows bult in firewall doesn't even bother looking at that.

What about Conficker that managed to get into systems remotely? With no software.

Conflicker spread through an exploit in Windows which was easy to patch if you just keep up to date with Windows Update. And again, systems behind a router weren't affected in the first place.
