If you mean a "blank" spot as in a white pixel, then no, not always. It depends what kind of fault is happening, it can be any color (or no color at all for that matter). A pixel "staying the same all the time" is more correct

Again, depends. It could well be a driver error too, or a refresh rate that isn't set up correctly. It is also possible that the connection between the monitor and PC is crappy and almost broken (same thing can happen on a PC when you don't plug in a monitor properly or the plug itself is broken). As last thing, it can indeed be a faulty screen too...
