I need a host that has the following:

50+ mb of room
500+ mb od bandwidth
Supports PHP
SQL support (1-2 databases)
Possibly CGI, SSI, CSS (external)
No forced ads
upload via FTP
simple url, eg: http://name.site.com or http://site.com/~name etc

It also must not have the following needed during sign up:

credit card
address/phone

unmetered bandwith (not unlimited, 30-40gig/month is probably the maximum the owner will give you, unless he likes you or your site ).

The bad side: you need 50 posts on the forums. That can easily be done in a day.
