This has happened to me when i was flodding someone with nudges who was using 6.2, then they blocked me, and after they unblocked me, that happed at the same time

Caused by a lag or the contact signed in and out to get attention.

The person could had been using the old version of Dimension, which has the feature toast-flooding

correction: The person has a mac not 6.2, he just changed so i didn't know. i dodn't think those things are made for mac...

It's probably just a server glitch or client glitch, maybe even a combination of both. Unless it happens again, it really doesn't matter much, but if it becomes a nuisance you could always try uninstalling MSN Messenger and reinstalling it, that usually stops small bugs like this.
