O.P. Window Problem

How do I fix this?

The Minimise, Maximise and Close buttons are now on the top and also the toolbar appears. How do I make it disappear, like how it is suppose to be.

Click "Help" and then uncheck "Show the menu bar"

I got the same problem again but on XP and there is no "Show the Menu Bar"
I'm on 8.5 final.
