i wouldn't know how but u'd have to reshack the msnmsgr.exe AFAIK....

You could use setparet api to put the button on the window, and then work out where to position it (which would be harder 
)

Btw, on that it tells you how to change msn today to 'add contact' with the correct icons etc. Not how to get it to open a site, And it does replace it. Not create a new one.
Im afraid you would have to figure that code out yourself....
