I think that the dark screen could be your screen backlight turning off. Might be because of a BIOS flaw like alby said.

There might be a broken connection somewhere. It could be in the cable, in the computer-end plug, in the monitor-end plug/entry, somewhere inside the monitor. I wouldn't but a new one without getting this one checked.

Not easily. I'll assume that your monitor has RGB input only, and that your PS2 has composite video output only, they can't be hotwired together.

Usually not, but a VCR in the middle usually will do conversion for you (no computer needed, but you will have to spend a bit). Some TVs/monitors have heaps of I/O ports, some have only analog RF in, most are somewhere in between.
