That icon is a the low quality 16 color version.
Each professional program (well, most of them) has several icons inside their resources. Each icon is of another quality and is used depending on your system settings. Eg: a 24bit alhpa channel, full color icon isn't going to work on a 16 color screen, therefore a 16 color icons is included in the executable....
Iether you probably beeped up your firefox executable (so it can't display the correct icon anymore). This can only be fixed by removing and reinstalling firefox. But since you say you tried that out already:
Or for some reason and if you're lucky, only the wrong icon is choosen, nothing more. This can be fixed, as buqingzi already said, by right clicking on it, choosing properties and changing the icon used to display the shortcut.
