Did you reinstall Wndows Media Player? I am leaning more towards my previous suspicion that the correct icons are being shown, but have been modified. The icon which you labelled "default" in your diagram is in wmplayer.exe, the main executable of Windows Media Player.
In case reinstalling Windows Media Player doesn't work, you can try using the wmplayer.exe from my computer, it's attached to this reply.
