You first need to get a userbar from a place like

Then you need to take that image and upload it to an image host like
http://imageshack.us
Then goto
http://shoutbox.menthix.net/usercp.php?action=editsig and type the code that they give under url for forum images.
Just copy paste it and you will see it
Thats just the basic way to get it up. If you want, you can add some text around it an stuff. If you want to see options such as center aligning and stuff then you might want to read the help topic
