RE: itunes plugin
Extract the plugin first and see if there's a readme wich usually contains instructions on how to install (the plugin either includes an installer or you'll have to place the dll file in the plugins folder, C:\Program Files\Messenger Plus! 3\Plugins, with MSN exited..and after that you restart MSN Messenger).

RE: itunes plugin
Some plugins might come as rar files. Download the file and open the file with an archive program that can deal with  RAR archives such as WinRAR (see http://www.rarlabs.com for an evaluation version of WinRAR). Then you can do as Sunshine says :)

RE: itunes plugin
If your plugin did not come with a readme, chances are it is simple enough to use without one. In that case, follow the instructions below.

Open a conversation window with a contact. Click the Plus! menu and click on or hover over the Plugins submenu. Click on or hover over the plugin's name and click the option you wish to use :)
